What is Vestibular Dysfunction?
Vestibular dysfunction is a condition where the inner ear or parts of the brain that help control balance and eye movements are not working properly. The vestibular system includes structures in the inner ear (like the semicircular canals and otolith organs) and their connections to the brain. When this system is disrupted, it can cause:
- Dizziness
- Vertigo (a sensation of spinning or movement)
- Balance problems or unsteady walking
- Nausea and vomiting
- Blurry vision or difficulty focusing
- Motion sensitivity (feeling worse with head movements or in busy visual environments)
Vestibular dysfunction can result from a variety of causes, such as inner ear infections, head injury, aging, migraines, or disorders like Ménière’s disease and vestibular neuritis.

If you’re experiencing sudden bouts of dizziness or the unsettling sensation that the room is spinning, you may be dealing with Benign Paroxysmal Positional Vertigo (BPPV) — a common condition caused by displaced crystals in the inner ear. Before beginning a Vestibular Rehabilitation program, many patients may first require Canalith Repositioning Therapy — a safe, non-invasive, and highly effective procedure designed to guide these crystals back to their proper position and help restore your sense of balance.
What Is BPPV?
BPPV occurs when tiny calcium crystals (canaliths) in your inner ear become dislodged and move into the semicircular canals, disrupting your body’s sense of balance. This can lead to brief but intense episodes of vertigo, especially with changes in head position.
What Is Canalith Repositioning?
Canalith Repositioning is a series of guided head and body movements that gently shift the displaced crystals back to their proper location in the inner ear, where they no longer cause symptoms. The most well-known of these is the Epley maneuver.
This treatment:
- Typically takes less than 30 minutes
- Provides relief after just one or two sessions in most cases

Vestibular Rehabilitation Therapy
Vestibular rehabilitation therapy (VRT) is a specialized, exercise-based treatment designed to help people manage dizziness, vertigo, balance problems, and gaze instability caused by vestibular (inner ear) disorders.

• Improve balance and postural control
• Enhance gaze (eye) stability during head movements
• Reduce dizziness and vertigo symptoms
• Increase the ability to perform daily activities

VRT uses exercises that promote the brain’s ability to adapt (vestibular adaptation) or compensate (substitution) for vestibular dysfunction. Key exercises include:
• Head and eye movement coordination
• Balance activities with different postures
• Gradual exposure to movements that provoke symptoms
• Functional tasks to improve daily living - VRT is effective for:
• Peripheral vestibular hypofunction (e.g., vestibular neuritis)
• Benign paroxysmal positional vertigo (BPPV)
• Stable vestibular lesions
• Dizziness in elderly individuals
• Some central disorders (e.g., after concussion)
Depending on the specific vestibular disorder and the patient’s individual symptoms, our therapists may use a combination of targeted techniques, including canalith repositioning maneuvers such as the Epley or modified Epley maneuver, alongside customized exercises like balance retraining, gaze stability drills, postural retraining, and gait training to support long-term recovery and improve overall stability.
